Prostate conditions often go unnoticed until they begin to significantly interfere with a man’s daily life, particularly with sexual activity and intimate relationships.

In this article, we explain how prostate conditions such as benign prostatic hyperplasia (BPH), prostatitis and prostate cancer can affect male sexual health. Understanding and caring for prostate health is a key step towards maintaining good sexual health and overall wellbeing.

What Is the Prostate and What Is Its Function?

The prostate is a gland approximately the size of a walnut, located just below the bladder in men, in front of the rectum. It is responsible for several functions related to sexuality and reproduction, including the production of seminal fluid, which helps improve sperm motility during fertilisation, and ejaculation, as the contraction of the prostate’s smooth muscle helps propel semen through the urethra.

In addition, although it is not its primary function, the prostate also plays a role in urinary flow control, as it surrounds part of the urethra and its size can influence the ease of urination.

What Are the Most Common Prostate Conditions?

The prostate can be affected by various conditions, some of which have a significant impact on male sexual health and quality of life. The most common are:

Benign Prostatic Hyperplasia (BPH).

Benign prostatic hyperplasia is characterised by enlargement of the prostate and is very common in men over the age of 60. Although it is not a malignant condition, it can compress the urethra and obstruct urinary flow, leading to symptoms such as weak urine stream, increased urinary frequency (especially at night), and a sensation of incomplete bladder emptying. While BPH itself does not directly affect sexual function, certain medical and surgical treatments may lead to erectile dysfunction or reduced sexual desire.

Prostatitis

Prostatitis refers to inflammation of the prostate and can present in several forms, including acute bacterial prostatitis, chronic bacterial prostatitis, chronic prostatitis/chronic pelvic pain syndrome, and asymptomatic inflammatory prostatitis. Symptoms vary depending on the type but may include pain during urination, pain in the genital or pelvic area, discomfort during or after ejaculation, and urinary symptoms similar to those of BPH. Prostatitis, particularly in its chronic form, can have a significant impact on sexual health, causing painful ejaculation and reduced libido.

Prostate Cancer

Prostate cancer is the most common cancer in men. In some cases, it grows slowly and remains confined to the prostate, causing limited harm. In other cases, it can be aggressive and spread rapidly. Symptoms may include urinary problems similar to those seen in BPH, blood in the urine or semen, erectile dysfunction, and bone pain. Because early symptoms are often mild or resemble those of other prostate conditions, early diagnosis is essential. Current diagnostic methods are far less uncomfortable for patients and significantly more accurate than in the past.

Treatment strategies for prostate cancer have evolved to improve cure rates while reducing treatment aggressiveness and minimising urinary and sexual side effects.

Impact of Prostate Conditions on Male Sexual Health

As outlined above, prostate conditions such as BPH, prostatitis and prostate cancer can significantly affect male sexual health, including erectile function, libido, ejaculation and overall sexual satisfaction.

Benign Prostatic Hyperplasia (BPH) and Sexual Health

Although BPH primarily affects urination, it can also impair sexual function directly or indirectly. Medical treatments and surgical procedures may lead to erectile dysfunction or retrograde ejaculation, a condition in which semen flows into the bladder instead of exiting through the penis. For this reason, treatment options are often selected with the aim of effectively managing BPH while preserving sexual function as much as possible.

Prostatitis and Sexual Health

Prostatitis, especially in its chronic form, can have a profound impact on sexual health due to persistent pelvic pain and discomfort related to ejaculation. These symptoms may reduce sexual desire and make intercourse uncomfortable or painful, leading to deterioration in intimate relationships and emotional distress.

Prostate Cancer and Sexual Health

Treatment for prostate cancer, including surgery, radiotherapy or hormonal therapy, may have a notable impact on sexual function, particularly erectile function and libido. Fortunately, significant advances have been made in recent years to achieve high cure rates while preserving male sexual health whenever possible.

Early Detection of Prostate Cancer

Until relatively recently, in addition to the stigma surrounding urogenital consultations among men, early detection of prostate cancer was often associated with uncomfortable or even painful diagnostic procedures.

Today, diagnostic approaches have changed substantially, offering greater accuracy, speed and minimal invasiveness. Specialists may use the following tools:

PSA Testing

This test measures the concentration of prostate-specific antigen (PSA) through a blood test. While it is not definitive on its own, elevated PSA levels may indicate underlying prostate conditions that require further investigation.

Multiparametric Magnetic Resonance Imaging (mpMRI)

Based on PSA results, the urologist may request a prostate MRI to obtain detailed images of the gland, confirm or rule out cancer, and assess its extent and possible spread.

Fusion Biopsy

This technique combines MRI and ultrasound imaging to accurately locate and analyse suspicious prostate tissue. It offers a detection accuracy above 97% for prostate cancer. In addition to minimising discomfort, it allows for more precise tumour characterisation and individualised treatment planning.

Treatments for Prostate Cancer

The main prostate cancer treatments currently used, while taking male sexual health into account, include:

Active Surveillance

This approach involves close monitoring of the patient to track tumour progression and initiate treatment at the appropriate time, while preserving urinary continence and erectile function. It is indicated for low-risk, slow-growing prostate tumours.

Surgery (Radical Prostatectomy)

This procedure involves removal of the prostate gland and is indicated when the tumour affects a large portion of prostate tissue. Robotic surgery, using systems such as Da Vinci, has significantly improved oncological and functional outcomes, allowing preservation of surrounding nerves and tissues to minimise urinary and sexual side effects.

Focal Therapy

Focal therapy treats only the tumour area plus a safety margin, without removing the entire prostate gland. It is used when the tumour is low-risk and confined to a single prostate lobe.

External Beam Radiotherapy

Technological advances now allow radiotherapy protocols to be delivered over five days or even in a single session, reducing treatment aggressiveness and helping preserve urinary and sexual function.

Managing the Impact of Prostate Conditions on Sexual Health

Although modern diagnostic and therapeutic approaches are increasingly minimally invasive and associated with fewer functional and sexual side effects, these may still occur in some cases. It is important to emphasise that effective management options are available, including oral or injectable medication, hormone replacement therapy, surgically implanted penile prostheses, and psychological support when appropriate.

Patients are encouraged to discuss any concerns openly with a urology specialist so that an individualised treatment strategy can be developed to address all aspects of their condition.

Lifestyle and Practical Advice to Support Prostate Health

Some key preventive measures to help reduce the risk of prostate conditions include:

Maintain a healthy lifestyle

A balanced diet rich in fruits, vegetables and whole grains, and low in saturated fats, together with regular physical activity, can support prostate health and reduce the risk of prostate disease.

Avoid tobacco use

Smoking increases the risk of many diseases, including prostate cancer. Quitting smoking can significantly reduce this risk.

Limit fluid intake before bedtime

For men with BPH symptoms, reducing fluid intake in the evening may help decrease nocturnal urination, improving sleep quality and overall comfort.

Attend regular medical check-ups

Even in the absence of symptoms, regular urology check-ups are recommended from the age of 50, or from 45 in men with a family history of prostate cancer or significant prostate conditions.

Pay attention to symptoms

Any unusual urinary or sexual symptoms should prompt consultation with a urologist, regardless of age. These include difficulty urinating, pelvic pain, blood in the urine or semen, and changes in sexual function.

We invite you to share this article on how prostate conditions affect male sexual health with anyone who may find it useful.

[et_pb_section fb_built=”1″ _builder_version=”4.17.6″ _module_preset=”0399be55-07e9-4778-8676-33277500c29f” global_colors_info=”{}”][et_pb_row _builder_version=”4.17.6″ _module_preset=”92704f8c-a4af-4de2-89c5-b36fbfb93efe” global_colors_info=”{}”][et_pb_column type=”4_4″ _builder_version=”4.17.6″ _module_preset=”default” global_colors_info=”{}”][et_pb_toggle title=”Referencias y autoría del contenido” content_last_edited=”off|desktop” admin_label=”Conmutador Blog” _builder_version=”4.23.1″ _module_preset=”8290cd9a-135d-4f2a-add0-c89f836c0146″ hover_enabled=”0″ locked=”off” global_colors_info=”{}” sticky_enabled=”0″]
This content has been prepared by the Communications Department and the Medical Team of Grupo Pedro Jaén, in accordance with our editorial commitment to accuracy and regular updates of the information provided.

References on prostate conditions and their impact on male sexual health:

1.
