They are often primarily an aesthetic concern, although in some patients they may coexist with underlying venous disease. For this reason, assessment should focus not only on their appearance but also on whether there are symptoms or signs of venous insufficiency.

What are spider veins?

Spider veins, also known as telangiectasia, are small red, purple or bluish blood vessels that are visible through the skin. They are usually superficial and most commonly appear on the legs, although similar vascular lesions may also be seen on the face and other areas.

Their development can be influenced by several factors, including genetic predisposition, hormonal changes, pregnancy, sun exposure and prolonged use of topical corticosteroids in some situations.

When spider veins occur in isolation, they are usually benign and treatment is often undertaken for cosmetic reasons.

However, numerous spider veins on the legs can sometimes coexist with chronic venous insufficiency, while facial telangiectasia may be associated with conditions such as rosacea. The clinical significance therefore depends on their location, distribution and associated symptoms.

What is chronic venous insufficiency?

Chronic venous insufficiency (CVI) occurs when the veins in the legs are unable to return blood efficiently towards the heart. This may result from dysfunction of the venous valves, obstruction or other abnormalities affecting venous blood flow.

Symptoms may include leg heaviness; swelling; aching; tingling; cramping; and discomfort after prolonged standing. Visible spider veins or reticular veins may be present, although their appearance alone does not establish the diagnosis of chronic venous insufficiency.

What are reticular veins?

Reticular veins are dilated bluish or greenish veins located slightly deeper beneath the skin than spider veins. They are generally larger than telangiectasia but smaller than typical varicose veins.

Some patients with reticular veins experience symptoms such as heaviness, aching, warmth or cramping, although others have no symptoms at all. Their clinical significance depends on whether they form part of a wider pattern of venous insufficiency or are associated with larger varicose veins.

Treatment of spider veins and reticular veins

Before starting treatment, it is important to carry out an appropriate medical history and vascular assessment. The examination helps determine whether the visible vessels are isolated or whether there is associated venous disease.

When indicated, a vascular specialist may request a Doppler ultrasound scan. This non-invasive test can assess venous blood flow and identify abnormalities such as valve dysfunction, thrombosis, obstruction or dilated veins that may not be apparent on physical examination alone.

Once the assessment has been completed, treatment can be tailored according to the type, size, depth and distribution of the vessels and the presence or absence of underlying venous disease.

For selected patients, treatment may involve sclerotherapy, vascular laser or a combination of both.

Sclerotherapy for reticular veins

Sclerotherapy involves injecting a liquid or foam sclerosant directly into the abnormal vein using a fine needle. The sclerosant damages the inner lining of the treated vessel, causing it to close and gradually be reabsorbed. Blood is then redirected through other functioning veins.

Sclerotherapy is commonly used for small varicose veins and reticular veins. The number of sessions required depends on the extent and characteristics of the vessels being treated.

It is generally performed as an outpatient procedure. Possible adverse effects include temporary bruising, pigmentation, local inflammation and, less commonly, other vascular complications.

After treatment, compression stockings may be recommended for a period determined by the specialist. Patients are also usually advised to follow individual aftercare instructions regarding physical activity and subsequent treatment sessions.

Laser treatment for spider veins

Vascular laser treatment can be useful for small superficial vessels, particularly when their size or location makes injection difficult.

The laser emits light that is absorbed primarily by haemoglobin within the blood. This produces controlled thermal damage to the target vessel, leading to its gradual closure and clearance. The number of sessions required depends on factors such as vessel size, depth, colour and extent. Laser is not necessarily the preferred treatment for every spider vein, and treatment choice should be based on the individual vascular pattern and clinical assessment.

Combined treatment

Spider veins and reticular veins often occur together. Because these vessels differ in size and depth, combined treatment with sclerotherapy and vascular laser may be appropriate in selected patients. For example, sclerotherapy may be used for larger reticular vessels and laser for finer superficial telangiectasia.

The final appearance develops gradually over the weeks following treatment. After vascular laser procedures, appropriate photoprotection is important to reduce the risk of pigmentation changes. Autumn and winter can be convenient times for these treatments because sun exposure is often easier to limit, although treatment can be carried out at other times of year when suitable precautions are followed.

The specialists in varicose vein treatment at Grupo Pedro Jaén’s Vascular Surgery Unit can assess the venous system, plan treatment and arrange appropriate follow-up, including the management of more significant venous disease when present.

Advice for reducing the risk of spider veins and varicose veins

Not all spider veins or varicose veins can be prevented. Genetic predisposition, age and hormonal factors cannot be modified.

However, certain measures can support general venous health and may help reduce symptoms associated with venous insufficiency:

avoid prolonged periods of complete immobility;

exercise regularly to support calf-muscle pump function and cardiovascular health;

stand up and move periodically if you spend long periods sitting; move the feet and ankles regularly during prolonged sitting;

maintain a healthy weight where appropriate; follow a balanced diet with adequate fibre to help prevent constipation;

elevate the legs when they feel swollen or heavy. These measures support circulation but cannot guarantee that spider veins or varicose veins will not develop.

We invite you to share this article about the treatment of spider veins and reticular veins with family and friends who may find it useful.

[et_pb_section fb_built=”1″ _builder_version=”4.17.6″ _module_preset=”0399be55-07e9-4778-8676-33277500c29f” global_colors_info=”{}”][et_pb_row _builder_version=”4.17.6″ _module_preset=”92704f8c-a4af-4de2-89c5-b36fbfb93efe” global_colors_info=”{}”][et_pb_column type=”4_4″ _builder_version=”4.17.6″ _module_preset=”default” global_colors_info=”{}”][et_pb_toggle title=”Authorship and references” content_last_edited=”off|desktop” admin_label=”Conmutador Blog” _builder_version=”4.17.6″ _module_preset=”8290cd9a-135d-4f2a-add0-c89f836c0146″ hover_enabled=”0″ locked=”off” global_colors_info=”{}” sticky_enabled=”0″]
The content of this article has been prepared by the Grupo Pedro Jaén Communications Department and Medical Team in accordance with our editorial commitment, which ensures the accuracy and regular updating of the information provided.

References on treatments for spider veins and reticular veins:

1.
