What it involves and how it is treated
A guide to ageing of the skin on the hands
Along with the neck and décolletage, the hands are one of the most commonly overlooked areas when it comes to skin ageing, despite being far more vulnerable to ageing factors and more exposed to external aggressions such as sun exposure, cold, wind, detergents, irritants, and repeated contact or friction with hard or abrasive surfaces.
The skin of the hands has very specific characteristics that must be taken into account when caring for and rejuvenating them. On the one hand, the skin of the palms is thick and highly resistant. It contains sweat glands and a thick layer of fat that acts as a protective cushion against trauma and friction.
In contrast, the skin on the back of the hands is much thinner and more delicate. It lacks sweat glands and contains very little hair and subcutaneous fat, meaning that its skin barrier (the hydrolipidic layer that protects the skin from external aggressions) is almost nonexistent. As a result, the hands often show signs of ageing much earlier than the rest of the body or even the face.
For this reason, the Grupo Pedro Jaén Aesthetics Unit has developed specific treatments for hand rejuvenation. The goal is to improve their appearance and harmonise the look of the hands with that of the patient’s face and body.
Treatments for pigmentation on the hands
Pigmentation is one of the most visible signs of ageing on the hands. It is caused by cumulative sun exposure over the years and, although it is more common in mature skin, it can also appear in younger patients who are frequently exposed to the sun without adequate protection.
Before starting any pigmentation treatment, it is essential for the dermatologist to assess the lesions in order to identify the type of pigmentation involved. This evaluation is crucial not only to select the most appropriate treatment, but also to rule out the presence of precancerous lesions or skin cancer.
The most effective treatments for pigmentation on the hands are the picosecond laser and intense pulsed light (IPL).
Picosecond laser
This is one of the most effective technologies for treating pigmentary disorders. It is particularly effective in removing dark brown spots without damaging the surrounding skin. In addition, its effect on collagen and elastin production improves skin quality and tone, achieving highly satisfactory overall hand rejuvenation.
Intense pulsed light (IPL)
IPL is a highly effective light source for treating sun-induced pigmentation. It is especially useful for hand rejuvenation because, in addition to reducing pigmentation, it also improves fine wrinkles. This allows IPL to address two very common concerns in ageing hands: sunspots and fine lines.
Pre- and post-treatment care
Before treatment with a picosecond laser or IPL, the dermatologist may recommend the use of a topical depigmenting agent for two to three weeks prior to the first session.
Laser and IPL treatments for hand pigmentation are performed in just a few minutes and do not require sedation or topical anaesthesia, as they are not painful. Patients can resume their normal daily activities immediately. However, it is essential to apply the restorative cream prescribed by the dermatologist and to use broad-spectrum sunscreen with a high protection factor (preferably SPF 50+) to optimise results and prevent post-inflammatory hyperpigmentation.
Combination of treatments for greater effectiveness
At the Grupo Pedro Jaén Laser Unit, we have the most advanced laser and IPL systems available to safely lighten hand pigmentation in the fewest possible sessions. These treatments can also be combined, at the dermatologist’s discretion, with topical depigmenting agents to inhibit melanin production and chemical peels to accelerate skin surface renewal, improving texture and overall appearance.
Treatments for wrinkles on the hands
The skin of the hands develops wrinkles earlier than that of the face or body due to the absence of sebaceous glands and the combined effects of ageing, sun exposure and environmental damage, which progressively impair the skin barrier. The most effective approach to treating hand wrinkles is based on combining fractional laser and/or intense pulsed light to stimulate collagen and elastin production through controlled heating of the skin. These structural proteins are essential for achieving firmer, smoother skin. One of the main advantages of laser and IPL hand rejuvenation is that results continue to improve progressively over the following months.
As with any laser or IPL treatment, patients must follow strict photoprotection measures using SPF 50+ sunscreen and apply the regenerative cream recommended by the dermatologist to support skin recovery.

Treatments to restore volume in the hands
Over time, the skin of the hands loses density and volume. This phenomenon of skin atrophy (thinning) gives the hands an aged appearance that may not correspond to the patient’s chronological age or facial appearance.
Depending on the degree of volume loss, different treatments may be indicated.
Mild-moderate volume los
If there is mild or moderate volume loss, mesotherapy can be used with moisturising active ingredients and resorbable fillers, as can regenerative techniques. These treatments redensify the hands by improving their hydration and stimulating the production of collagen and elastin, substances linked to skin firmness.
Severe volume loss
When the hands appear excessively bony and hollow, resorbable fillers, collagen stimulators and autologous plasma (PRP) are recommended. These substances restore volume through their water-retention capacity and their ability to promote collagen production. The volumising effect is immediate, and skin texture and appearance continue to improve over the following weeks. These treatments are very safe, as they are based on biocompatible materials that are gradually reabsorbed and can be safely repeated once fully degraded.
A special mention should be made of hand rejuvenation using autologous fat, which is increasingly considered an ideal filling material due to its natural, long-lasting results and excellent safety profile.
This technique involves harvesting a small amount of fat via minor liposuction from areas such as the abdomen or knees. The fat is then processed and injected into the hands to restore volume, improve vascularisation and enhance skin tone. The rejuvenating effect is natural, safe and long-lasting.
