Treatments designed to enhance the appearance of the lips have become one of the most frequent requests among patients attending our Facial Aesthetics Unit. This growing demand is linked to evolving beauty standards, as well as to an increasing number of younger patients seeking to improve the appearance of their lips through non-invasive procedures that deliver harmonious and natural-looking results.

The lips and the perioral area can present very different needs depending on each patient. For this reason, specialists at the Grupo Pedro Jaén Facial Aesthetics Unit consider a personalised assessment essential. This evaluation goes beyond analysing lip shape and skin quality; it also takes into account facial proportions and symmetry in relation to the chin, nose, cheeks, cheekbones and even the forehead, in order to avoid disproportionate or artificial-looking results.

As a result, treatment selection is tailored to each individual, and in many cases, several therapeutic options are combined to achieve comprehensive aesthetic improvement.

Main aesthetic treatments for the lips

Intensive and long-lasting hydration

The injection of small amounts of a substance with a high capacity to retain water directly into the lip mucosa and vermilion border provides immediate rehydration and revitalisation. The result is smoother, more supple lips with fewer fine lines, achieved instantly and without adding volume. The lips retain their natural shape and are not enlarged; they simply appear healthier, better coloured and more youthful.

Lip contouring and definition

Over time, the natural contour of the lips tends to lose definition. To restore and refine their shape, a filler specifically designed for the lip area is injected using microcannulas, allowing precise placement exactly where it is needed. The result is immediate and very natural, without adding extra volume—only improving the lip outline. This technique is also used to redefine the Cupid’s bow, which often flattens with age.

Lip flip

The lip flip is one of the most popular techniques for rejuvenating the lips and enhancing the appearance of the mouth in a subtle and natural way, without increasing lip volume. It involves injecting small doses of a neuromodulator into the orbicularis oris muscle (also known as the “kissing muscle,” responsible for puckering the lips), reducing excessive contraction.

This technique not only softens early fine lines that may later develop into so-called “barcode lines,” but also causes a slight outward rotation of the upper lip. This creates a fresher, more youthful appearance without affecting speech, facial expression or lip volume. When increased fullness is desired, the lip flip can be combined with dermal fillers.

Perioral vertical lines (“barcode lines”)

These vertical wrinkles above the upper lip are among the most aesthetically impactful perioral concerns. They are particularly common in smokers, individuals with pronounced facial expressions or those with significant sun exposure. In early stages, their progression can be slowed and softened using a combination of lip flip techniques and small amounts of dermal fillers to smooth the most pronounced lines.

When these wrinkles are more established, treatment is usually combined with rejuvenation laser therapies to renew the skin surface and stimulate collagen and elastin production. This improves skin firmness and texture, resulting in a smoother and more resilient perioral area.

Lip eversion technique (Russian lips)

This technique requires a highly refined approach to increase lip volume while avoiding unnatural results such as the “duck lip” appearance. To achieve harmonious outcomes, fillers are injected deeply within the lip mucosa using very fine cannulas, allowing the lip to rotate slightly outward without projecting forward. The result is more striking than other lip treatments and is designed to be noticeable. The key lies in thorough patient counselling and precise dosing, ensuring a balance between enhanced volume and overall facial harmony to avoid an oversized or artificial appearance.

Gummy smile

Aesthetic lip treatments are not limited to enhancing lip appearance; they can also correct issues that affect overall facial harmony, such as asymmetries or a gummy smile. In some patients, there is a significant imbalance between the upper and lower lips—most commonly a much thinner upper lip—which can be corrected using fillers to restore proportion

In cases of gummy smile, where excessive gum is visible when smiling, neuromodulator injections are used to reduce the overactivity of the muscles that elevate the upper lip excessively.

Upper lip lift (lip lift)

The upper lip lift is a minimally invasive surgical technique designed to shorten the philtrum (the distance between the base of the nose and the upper lip) in patients who show little or no upper teeth when speaking or smiling—the opposite of a gummy smile. The goal is to achieve a more balanced and aesthetically pleasing expression.

The procedure involves a small incision at the base of the nose, which is later well concealed. By removing a small amount of skin, the upper lip is subtly elevated and rotated outward, resulting in a comprehensive improvement of the entire perioral area.

Complementary treatments

As noted above, dermatology and aesthetic medicine specialists do not focus solely on the specific concern expressed by the patient during consultation. Achieving harmonious results requires a global assessment of the face, including facial movement, resting volume and shape, and potential excess skin. For this reason, lip treatments are often complemented by additional procedures, such as the correction of nasolabial folds (creases running from the nose to the corners of the mouth) or marionette lines (vertical lines extending from the mouth corners towards the chin).

Nasolabial folds are typically treated with denser fillers than those used in other facial areas, while marionette lines respond particularly well to fractional CO₂ laser treatment. This technology renews the skin surface and stimulates collagen and elastin production, leading to a tightening effect and a significant improvement in skin quality, noticeably softening these wrinkles.

Finally, our Smile Aesthetics Unit offers further enhancement of the lower third of the face through dental whitening and/or porcelain veneers.
