A guide to understanding scalp micropigmentation

The aim of scalp micropigmentation (also known as tricopigmentation) is to recreate the appearance of shaved hair in areas where hair is absent, restore the frontal hairline or simulate greater hair density in areas of thinning. In addition, scalp micropigmentation is also used to camouflage scalp scars caused by trauma or by scarring alopecias in which hair regrowth is no longer possible.

This technique is a suitable alternative for individuals who do not wish to undergo a hair transplant, for those who are not good candidates due to specific contraindications, for patients undergoing medical treatment for alopecia who wish to improve their appearance while waiting for pharmacological results, or for those who have reached the maximum benefit after hair transplantation or medical therapy and require an additional aesthetic enhancement.

Techniques and indications for scalp micropigmentation

Scalp micropigmentation includes different techniques depending on the desired visual effect. These include:

• Stippling. This technique involves applying tiny pigment dots that imitate hair follicles, recreating the appearance of shaved hair. It is commonly used in patients with extensive areas of hair loss or in cases of universal alopecia.

• Shading. Pigment is applied to create a diffuse shadow, producing a visual effect of increased hair density.

• Camouflage. This technique is used to conceal scars resulting from trauma, previous surgery or hair conditions that have left visible marks on the scalp. In these cases, pigments matching the surrounding skin tone are used and blended with follicular pigmentation so that the scar adopts the same colour as the adjacent tissue.

These techniques may be applied individually or in combination, depending on each patient’s needs. The appropriate approach is determined after an initial assessment and allergy testing, which is carried out to minimise the risk of adverse reactions during the micropigmentation process.

Procedure and duration of results

Once the treatment area has been defined and the techniques to be used have been established, an initial session is performed. This is followed by a second session after approximately 15 days and a third session one month after the previous treatment.

The duration of scalp micropigmentation sessions typically ranges from one to three hours, depending on the size of the area treated and the technique applied.

No specific recovery measures are required after the sessions, although patients are advised to avoid sun exposure and to refrain from touching, scratching or wetting the treated area during the first 24 hours.

The results of scalp micropigmentation—performed at a depth greater than microblading but more superficial than tattooing—usually last for around three years. However, durability may vary depending on factors such as skin type and perspiration.

Over time, the pigment gradually fades. When necessary, the treatment can be repeated, according to the patient’s needs and the specialist’s recommendation.

Finally, as this is a superficial procedure, scalp micropigmentation is not painful. Some patients may experience mild discomfort, which can be effectively minimised with topical local anaesthesia when required.
