A guide to understanding brow lift surgery

The eyebrows, eyes and periocular area form a unit that plays a decisive role in facial aesthetics. Drooping eyebrows, dark circles, under-eye bags, forehead and glabellar wrinkles, or excess eyelid skin can give the face a tired, aged, sad or even angry appearance — often at surprisingly young ages.

In addition to aesthetic results, particular attention must be paid to functional safety.

In the upper third of the face, brow lift surgery is a minimally invasive surgical option designed to reduce wrinkles, open up the gaze and lift the tail of the eyebrow and surrounding tissues that have descended over time.

What does a brow lift involve?

Brow lift surgery is not a new technique. It has been performed for many years, traditionally as part of a full facelift. The Grupo Pedro Jaén Facial Aesthetics Unit has incorporated the brow lift as an independent procedure for patients who are not suitable candidates for medical-aesthetic treatments performed in the clinic, but who do not yet require a full facial lift.

A brow lift is a minimally invasive procedure that consists of lifting the tail of the eyebrow through a very small incision hidden within the hair-bearing area of the temple, usually three to four centimetres behind the hairline. The surgeon then gently releases the skin, applies traction to the frontalis muscle via the superficial fascia, and places suspension sutures in this superficial fascia to ensure a long-lasting result.

This is a fully outpatient procedure, performed under local anaesthesia, with a very short recovery period and no need for complex post-operative care, as inflammation is usually minimal. In fact, most patients return to their normal daily routine within four to five days.

Results of brow lift surgery

The result of this minimally invasive procedure is a more open, refreshed and youthful gaze. In addition to lifting the eyebrows and immediately eliminating the tired appearance associated with brow descent, the procedure also smooths pronounced wrinkles on the forehead, around the eyes and between the eyebrows.

A brow lift is an excellent option for patients who have limited alternatives with medical-aesthetic treatments, who do not yet wish to undergo a full facelift, or for whom a more invasive procedure is not necessary because the problem is localised to the upper third of the face.

Indications for brow lift surgery

A brow lift is indicated in patients with eyebrow ptosis (drooping eyebrows), either as an isolated concern or associated with excess skin of the upper eyelids.

Traditionally, the desired effect in the upper third of the face has often been sought through injectable treatments that relax the musculature to open the gaze and lift the tail of the eyebrow. However, a brow lift is a more effective solution when these treatments are insufficient or not indicated, such as in cases of drooping caused by excess tissue.

In all cases, the goal of brow lift surgery is to subtly open the gaze and restore a rested expression, without giving the patient a sad or angry appearance. The procedure is never intended to change the individual’s expression or facial identity.

This treatment is suitable for both men and women. Although age is not a decisive factor, brow lift surgery is most commonly performed in patients between approximately 45 and 65 years of age.

Recovery after brow lift surgery

As a minimally invasive outpatient procedure, recovery after a brow lift is generally quick and associated with very little discomfort or specific post-operative requirements.

Pain and swelling are usually mild and can be effectively managed with local cold application and commonly used analgesics.

Patients can resume normal daily activities within a few days, taking care to avoid applying make-up to the treated area and to follow simple home wound care for four to five days (gentle cleansing with saline solution, disinfection with chlorhexidine and coverage with a sterile dressing).

Smoking is strictly contraindicated, as it interferes with proper tissue healing.

Recovery times and specific post-operative instructions may vary from one patient to another, particularly if the brow lift has been performed in combination with upper eyelid blepharoplasty or depending on individual clinical needs.
