Blepharoplasty is also recommended in cases where excess skin on the upper eyelid causes health problems, such as a reduced field of vision. This surgery requires great surgical precision and experienced specialists. They carry out a thorough anatomical assessment and plan an intervention using minimally invasive techniques to reduce trauma to the eye area. The ultimate goal is to harmonise the expression of the eyelids with the rest of the face and to foster a fresh and revitalised appearance while protecting the eyelid function.
What it involves, indications and outcomes
A guide to understanding blepharoplasty (eyelid surgery)
Blepharoplasty is one of the most rewarding procedures within facial aesthetic surgery and one of the most frequently requested by both men and women from the age of 45 onwards. By addressing the eyelids and under-eye bags alone, it can achieve a rejuvenating effect on the entire face. The procedure may be performed on the upper eyelids, the lower eyelids or both, depending on each patient’s needs. It can also be combined with other treatments to achieve a comprehensive improvement of the periocular area, such as brow lifting, laser rejuvenation, chemical peels or injectable medical aesthetic treatments.
Indications for blepharoplasty
Blepharoplasty is designed to rejuvenate the periocular area by removing excess eyelid skin caused by tissue laxity associated with ageing, as well as under-eye bags resulting from localised fat accumulation. Although this procedure is mainly performed for aesthetic reasons, it may also be indicated for functional purposes, as drooping eyelids can sometimes interfere with the patient’s field of vision.
The procedure must be performed by experienced surgeons
It is essential that blepharoplasty is carried out by experienced surgeons with in-depth knowledge of the anatomy and function of the periocular area, ensuring that the aesthetic intervention does not negatively affect vision. This surgery should enhance facial appearance without altering expressiveness. The aim is to achieve a fresher, more rested and more youthful look, not to change the shape of the eyes or modify the patient’s facial expression.
Blepharoplasty may appear to be a straightforward procedure, but it requires a thorough understanding of periocular anatomy and function in order to achieve optimal results while minimising risks and potential complications. To obtain harmonious outcomes, it is crucial to be treated by surgeons with extensive experience and aesthetic sensitivity. In the upper eyelid, the levator muscle—responsible for eyelid movement—is located in this area, meaning that surgery must be performed with great precision to avoid damage.
Similarly, lower eyelid blepharoplasty aimed at removing excess skin can result in excessive eyelid drooping if performed too aggressively, leading to an overly open eye or incomplete eyelid closure (ectropion). In addition to being aesthetically undesirable, this can impair ocular surface lubrication during blinking, potentially causing significant problems related to excessive dryness, such as irritation, pain, a gritty sensation, blurred vision or corneal ulcers.
What the surgery involves and main types of blepharoplasty
This procedure is performed under local anaesthesia with sedation and is usually carried out on an outpatient basis. Depending on the extent of the intervention, blepharoplasty typically lasts between one and two hours, or slightly longer if combined with additional procedures such as a brow lift. Post-operative bruising usually resolves within 5 to 10 days, although recovery can be accelerated using a specific type of laser available in our Laser Unit.
Blepharoplasty is associated with a high level of patient satisfaction due to its favourable aesthetic results, which become definitive approximately two to three weeks after surgery.
Upper blepharoplasty
This operation removes that excess skin from the upper eyelids which can give the patient a tired or sad appearance or can even affect vision. The incision is marked in the natural crease, ensuring a discreet scar that will be virtually unnoticeable. As necessary, skin, fat or part of the orbicularis oculi muscle is removed and the eyebrow height is assessed for a rejuvenating and functional result, without altering the natural expression.
Lower blepharoplasty
This surgical procedure is performed to remove excess skin from the under-eye area and is the most effective option for eliminating under-eye bags. When these bags are small and mainly caused by fluid retention, they may be improved using aesthetic treatments and devices available in our Facial Aesthetic Medicine Unit and Beauty and Wellness Unit. However, when bags are pronounced or caused by fat accumulation, lower blepharoplasty is required.
The procedure is performed via a transconjunctival incision (through the conjunctiva, the transparent membrane lining the eye), meaning that the skin surface does not need to be incised.

Blepharoplasty outcomes
The result of eyelid surgery is a more open, rested and refreshed appearance. The effects are completely natural and do not involve any change in eye shape or facial expression. The objective of this procedure is not to transform facial features, but to correct a sign of ageing that may, in some cases, also have functional implications. In fact, many patients report an improvement in vision after excess eyelid skin is removed, particularly when it previously interfered with their visual field.
The benefits of surgery become apparent quickly, as inflammation is temporary and minor bruising typically resolves spontaneously within 5 to 10 days, depending on the extent of the procedure. Final results are usually visible around one month after surgery, although patients can generally resume normal daily activities a couple of weeks earlier.
During the first few days, patients should avoid using make-up or cosmetic products around the eyes. Physical exercise should be avoided for at least two weeks. Patients are also advised to sleep on their back to prevent pressure or friction from the pillow, and to apply strict sun protection using appropriate filters and sunglasses for the period recommended by the surgeon.
Complementary treatments for blepharoplasty
Blepharoplasty alone provides excellent results. However, to achieve a more comprehensive rejuvenating effect, it can be complemented with injections of neuromodulators to soften crow’s feet and frown lines, laser rejuvenation, ultrasound, radiofrequency or chemical peels to improve brightness, texture and skin quality in the periocular area.
