Types, location and treatment options

A guide to understanding facial wrinkles

Facial wrinkles are one of the most visible signs of skin ageing. In addition to chronological ageing, their development is influenced by genetic and hormonal factors, as well as environmental and lifestyle-related factors such as excessive sun exposure, smoking, cosmetic habits and overall lifestyle.

It is also important to distinguish between the different types of wrinkles that appear on the face, particularly dynamic wrinkles (also known as expression lines) and static wrinkles (associated with the progressive loss of skin elasticity that occurs with ageing). The nature and origin of facial wrinkles largely determine the most appropriate treatment approach to reduce their appearance.

Expression lines

Expression lines, or dynamic wrinkles, are directly related to facial movement and muscle activity. They develop when the skin repeatedly folds as a result of contraction of the facial musculature. Although they are exacerbated by the gradual loss of skin elasticity that occurs with age, expression lines are not necessarily age-related.

In fact, some individuals develop expression lines at a young age for a variety of reasons, which may negatively affect facial appearance and give the face a tired, sad or prematurely aged look.

Location and characteristics of expression lines

Expression lines are most commonly found in the following areas:

Forehead

Forehead expression lines typically appear as horizontal lines across the forehead. They are mainly caused by repeated eyebrow elevation and may appear clustered above the eyebrows or extend across the entire forehead. They are more common in highly expressive individuals and tend to become more pronounced over time.

Glabellar area (between the eyebrows)

Glabellar lines form vertically between the eyebrows due to the action of the corrugator muscles. Also known as superciliary muscles, these small muscles are located obliquely in the eyebrow region and are involved in frowning and movement of the upper eyelids. Depending on the strength and symmetry of muscle contraction, glabellar lines may appear as paired lines or be more marked on one side.
As with other expression lines, glabellar wrinkles tend to deepen with age. However, they are also very common in younger individuals who frequently frown, such as people with myopia who squint to see at a distance, patients with photophobia or marked light sensitivity, or individuals with very light-coloured eyes.

Crow’s feet

Periocular wrinkles, commonly known as crow’s feet, often appear relatively early due to two main factors: facial movement and the specific characteristics of the skin around the eyes.
Crow’s feet are linked to the action of the orbicularis oculi muscle, a ring-shaped facial muscle that surrounds the eye and is involved in blinking (specifically eye closure) as well as in what is known as a genuine smile. This muscle is activated during smiling and laughter, while remaining relatively inactive during a social or forced smile. These wrinkles tend to appear earlier and be more pronounced because the skin around the eyes is thinner and contains lower levels of collagen and elastin, making it more vulnerable to dehydration and loss of firmness.

Nasolabial folds

Also known as smile lines, these expression lines extend from the base of the nose towards the corners of the mouth, either obliquely or vertically. They become more noticeable when smiling, which is why they are often visible from an early age without necessarily causing an aesthetic concern.
However, cumulative sun damage and age-related skin laxity contribute to their progressive deepening, which can eventually lead to so-called marionette lines, giving the face a downturned or tired appearance.

Perioral area

Wrinkles in this area, commonly referred to as “barcode lines”, are fine vertical expression lines that appear around the mouth, particularly on the upper lip. They are mainly caused by frequent and forceful pursing of the lips, although they are significantly aggravated by smoking, excessive sun exposure and inadequate hydration of the area.

Other areas

Other facial regions may also develop expression lines, including the sides of the nose, the chin and the vertical bands of the neck. These wrinkles result from exaggerated facial movements during speaking or gesturing. Although they tend to worsen over time, they are not necessarily a direct consequence of normal ageing.

Treatments for expression lines

Injectable treatment

The most effective treatment for expression lines involves the injection of small doses of a medication that relaxes the facial muscles, preventing excessive contraction and the subsequent formation of wrinkles on the skin surface. This medication acts in a highly targeted manner at the injection site and does not affect surrounding tissues.

The goal of this injectable treatment is to prevent wrinkle formation while preserving natural facial expression. Achieving this requires an in-depth understanding of facial anatomy and precise injection techniques tailored to each treatment area.

This is a very safe procedure, performed in the clinic and typically completed in around 20 minutes. As with any injectable treatment, mild discomfort or small pinpoint bleeding at the injection site may occur, although these effects are minimal and do not require specific medical treatment. Patients can resume normal activities immediately. The effects usually begin to appear within 24 to 48 hours, with full results becoming visible approximately two weeks after treatment. In most cases, a follow-up visit is scheduled to assess the outcome and make any necessary adjustments.

The result is a refreshed appearance with relaxed, natural facial expressions that are both subtle and harmonious.

The effects of injectable treatment for expression lines typically last between four and six months, depending on the treated area. Treatment can be repeated once this period has elapsed. Clinical experience has shown that better results are achieved with smaller doses administered more frequently, rather than higher doses spaced further apart.

Other treatment options

There are additional options for reducing expression lines that focus on improving skin quality by stimulating collagen and elastin production. These include biostimulation using regenerative medicine techniques, radiofrequency treatments, dermal fillers and collagen stimulators, as well as laser skin rejuvenation. In most cases, these treatment options are combined with injectable muscle-relaxing therapy to achieve a more comprehensive and balanced rejuvenation outcome.

Static wrinkles

Static facial wrinkles are associated with a deterioration in skin quality and the natural loss of skin volume, commonly referred to as skin laxity. Skin laxity is characterised by reduced firmness and sagging of the tissues due to the progressive decline in collagen and elastin that occurs naturally over time. This process usually begins around the age of 35 and becomes more pronounced from approximately 40 years of age onwards.

However, the normal ageing and laxity process may be significantly exacerbated by external factors such as cumulative sun damage, skin dehydration, smoking, lack of sleep, and certain skin conditions, including psoriasis or atopic dermatitis. All of these factors can negatively affect overall skin health and contribute to the development of wrinkles.

Static wrinkles tend to appear in areas where facial movement and expression have led to deep crease formation over time, such as the forehead or nasolabial folds. They may also affect the neck, the lower third of the face and the perioral region.

Treatments for static wrinkles

Treatment of static wrinkles is based on filling deep lines and folds, as well as stimulating collagen and elastin production through different therapeutic approaches. The aim is to restore volume, improve skin support and reduce wrinkles associated with skin laxity.

Collagen is a structural protein that plays a key role in the integrity and strength of supporting tissues and is therefore essential for skin density and firmness. Elastin, on the other hand, is responsible for skin elasticity and flexibility, allowing the skin to recover its shape despite the repeated movements, gestures and mechanical stress it is exposed to on a daily basis.

The injection of resorbable dermal fillers involves administering small amounts of a biocompatible material widely used in various medical fields due to its ability to retain water. Depending on its formulation and density, this substance can be used to improve skin hydration, fill deep lines and wrinkles, or restore volume in selected areas.

Collagen stimulators, in turn, are resorbable substances injected into targeted areas to stimulate the production of collagen and elastin, helping to reduce skin laxity and thinning, which are responsible for static wrinkles.

hese treatments are performed in the consultation setting and are administered using needles or cannulas of different calibres. Their effects last for a variable period ranging from six months to two years, depending on the substance and its formulation. Once this period has elapsed and the material has been fully reabsorbed by the body, the procedure can be safely repeated.

As with expression lines, static wrinkles can be treated using different therapeutic combinations to achieve optimal results.
